Program objectives
- To enable pharmacist/clinical pharmacists to establish, implement, deliver, and sustain an MTM service at their institution
- To develop and enhance essential clinical and communication skills, and pharmacotherapy knowledge needed for performing MTM successfully
- To provide a roadmap for identifying medication-related problems, how to address it within a multidisciplinary team and participate in improving medication safety
- To improve medication utilization through creating an optimal medication therapy plan tailored to individual patient’s needs
- To ensure patients participation and adherence to their health care success
What Will I Learn?
- A- Definition of Medication Therapy Management (MTM): it is a distinct service or group of services that optimizes therapeutic outcomes for individual patients
- B- Aim of MTM services: enhancing patients’ understanding of appropriate drug use, increasing adherence to medication therapy, and improving detection of adverse drug events.
- C- The five core components of an MTM service:
- Medication therapy review (MTR).
- Personal medication record (PMR).
- Medication-related action plan (MAP).
- Intervention and/or referral.
- Documentation and follow-up.
