Program objectives

  1. To enable pharmacist/clinical pharmacists to establish, implement, deliver, and sustain an MTM service at their institution
  2. To develop and enhance essential clinical and communication skills, and pharmacotherapy knowledge needed for performing MTM successfully
  3. To provide a roadmap for identifying medication-related problems, how to address it within a multidisciplinary team and participate in improving medication safety
  4. To improve medication utilization through creating an optimal medication therapy plan tailored to individual patient’s needs
  5. To ensure patients participation and adherence to their health care success

What Will I Learn?

  • A- Definition of Medication Therapy Management (MTM): it is a distinct service or group of services that optimizes therapeutic outcomes for individual patients
  • B- Aim of MTM services: enhancing patients’ understanding of appropriate drug use, increasing adherence to medication therapy, and improving detection of adverse drug events.
  • C- The five core components of an MTM service:
  • Medication therapy review (MTR).
  • Personal medication record (PMR).
  • Medication-related action plan (MAP).
  • Intervention and/or referral.
  • Documentation and follow-up.
