Course Objectives & Learning Skills

Objectives:

  1. To enable clinical pharmacists to establish, implement, deliver, and sustain an MTM service at their institution
  2. To develop and enhance essential clinical and communication skills, and pharmacotherapy knowledge needed for performing MTM successfully
  3. To provide a roadmap for identifying medication-related problems, how to address it within a multidisciplinary team and participate in improving medication safety
  4. To improve medication utilization through creating an optimal medication therapy plan tailored to individual patient’s needs
  5. To ensure patients participation and adherence to their health care success

 

 

Learning skills:

Further to this course, it is recommended that the candidate expand to the following

  1. Medication therapy management (MTM) core elements
  2. Chronic disease management (CDM)
  3. Collaborative practice agreement
  4. Evidence-based medicine (literature review)
  5. Basic skills for obtaining point of care lab/vital signs
